If you are interested in our financial assistance program you can download the application form below. Completed applications can be mailed or brought to Sherman Hospital.

Uninsured Discount

If you are uninsured and meet certain income requirements you may qualify for a discount through the Uninsured Discount Act- Senate Bill 2380. The application process is the same as our financial assistance program.
